Terms of Service

You must be at least 16 years old to use the Service. By using the Service, you represent that you meet this requirement and that all information you provide is accurate.

FORMA analyzes photos you provide to generate skin and facial metrics, reports, and a personalized improvement protocol. FORMA is not a medical device, and its scans, reports, and AI coaching conversations are not medical advice, diagnosis, or treatment. Always consult a licensed dermatologist, physician, or other qualified professional for medical concerns. If a scan flags something that concerns you (for example, a detected lesion), treat it as a prompt to seek a professional opinion, not as a diagnosis.

You're responsible for maintaining the confidentiality of your account credentials and for all activity under your account. Let us know immediately at support@tryformaai.app if you suspect unauthorized use of your account.

You retain ownership of the photos and any other content you submit to the Service ("Your Content"). By uploading Your Content, you grant FORMA a limited, non-exclusive license to store, process, and analyze it solely to provide and improve the Service for you, as described in our Privacy Policy. We do not sell Your Content or use it for advertising.

You're responsible for making sure you have the right to upload any photo you submit, and that it depicts you or someone who has given you permission.

FORMA's AI coaching specialists generate guidance based on the information you provide and third-party AI processing (see our Privacy Policy for details). Responses are generated automatically and may occasionally be inaccurate, incomplete, or not applicable to your situation. Use your own judgment before acting on any suggestion.

Some features require a paid subscription, purchased through Apple's App Store using In-App Purchase and managed on our behalf by RevenueCat. Subscriptions automatically renew at the end of each billing period unless canceled at least 24 hours before renewal.

• You manage and cancel subscriptions through your Apple ID account settings, not through FORMA directly.
• Refunds are handled by Apple according to the App Store's refund policies — FORMA does not process refunds directly.
• We may change subscription pricing or features going forward; changes won't apply retroactively to a period you've already paid for.

We may make pre-release versions of the app available through Apple's TestFlight program before a wider release. TestFlight builds may be incomplete, may change or be removed without notice, and may contain bugs. We appreciate — and actively rely on — feedback from TestFlight testers; you can send it any time to support@tryformaai.app.

You agree not to:

• Upload photos of anyone without their consent
• Use the Service for any unlawful purpose or to violate anyone's rights
• Attempt to reverse-engineer, scrape, or extract our models, reports, or underlying data at scale
• Interfere with or disrupt the integrity or performance of the Service
• Share your account with others

You may stop using the Service and delete your account at any time (see our Privacy Policy for how to request deletion). We may suspend or terminate your access if you violate these Terms or misuse the Service.

The Service is provided "as is" and "as available," without warranties of any kind, express or implied. To the maximum extent permitted by law, FORMA will not be liable for any indirect, incidental, special, or consequential damages, or for any loss of data, arising from your use of the Service. Our total liability for any claim relating to the Service will not exceed the amount you paid us in the twelve months before the claim arose.

We may update these Terms from time to time. If we make material changes, we'll notify you by email or through the Service before the changes take effect. Continued use of the Service after changes take effect means you accept the updated Terms.

These Terms are governed by the laws of Canada, without regard to conflict-of-law principles.
